Scheduling Your Screening Mammogram

It's easier than ever to schedule your routine annual screening mammogram at Baptist Health.

You have a choice of two convenient ways to schedule your appointment:

1. Online form - Click on the type of test you need and fill out a form online to request your appointment. A Central Scheduling representative will contact you with the date/time/location of your visit along with any prep instructions.
   • Screening Mammogram
   • Screening Mammogram and Bone Density Study (A physician's written referral is required for a Bone Density Study.)
2. Phone Call - Call one of our friendly Central Scheduling representatives at 904.202.2222.
   8 am - 5:30 pm, Monday - Thursday
   8 am - 4:30 pm on Fridays
